In this role, you will maintain direct service for a designated number of children, writing and keeping current case notes and files as well as participate in weekly team meetings and all scheduled in-service transdisciplinary meetings. You will participate in a transdisciplinary assessment and evaluation of designated children using a designated assessment tool and act as the liaison between children, families, and local community agencies to provide support for families. You will coordinate 6-month reviews, annual evaluations, and new IFSPs for all assigned children.
