We focus on health equity by developing programs and systemic solutions that reduce disparities for the people and communities we serve.\rAlameda County Health’s departments and programs focus on services and support that provide care for the whole person.\rThe Behavioral Health Department provides mental health and substance use services for people with Medi-Cal and without insurance and supports people along their path to wellness, recovery, and resilience.\rThe Environmental Health Department works to keep our air, water, and food safe; it regulates, protects, and promotes the health of everyone in Alameda County by enforcing environmental health codes to reduce exposure to toxins and diseases.\rThe Public Health Department focuses on community and population-level health, preventing and addressing root causes of health inequity across a range of communicable and chronic diseases.\rAlameda County Health also provides services through Housing and Homelessness Services, Emergency Medical Services Agency, HealthPAC, and Healthy Schools and Communities.\r\r\r\r\rPUBLIC HEALTH DEPARTMENT\rAs part of Alameda County Health, the Public Health Department works in partnership with our local communities to ensure the optimal health and well-being of all people. We monitor health status and service delivery, prevent disease, mobilize communities, conduct outreach, and promote health policy and education. We actively seek partnerships and engage with the community to improve community health through a dynamic and responsive process that respects diversity, addresses health equity, and challenges us to provide for present and future generations.\r\rWe are nurses, doctors, community psychologists, epidemiologists, dentists, medical social workers, physical and occupational therapists, dietitians, outreach workers, health educators, program managers, and pre-hospital care coordinators.\rTHE POSITION\r\rUnder\rgeneral supervision, to perform program-specific, evidence-based, long-term\rcase management in the Health Care Services Agency (HCSA) case management, care\rcoordination, home visiting and/or family support programs and work\rcollaboratively to provide a seamless system of services and support for\rclients in Alameda County; acts as a liaison between communities, agencies, and\rother resources, services and program staff; and to do related work as\rrequired. Program-specific, evidence-based case management is defined as\rintensive work with families over a period of two to three years using\revidence-based models specific to the program which incumbent is assigned and\rtarget client population(s). However, the eligible list\rresulting from this recruitment may be used to fill future vacancies within\rother program areas.\r\rMATERNAL, PATERNAL, CHILD, AND ADOLESCENT HEALTH (MPCAH) UNIT\rMaternal, Paternal, Child, and Adolescent Health (MPCAH) is dedicated to\rcoordinating services for families, including teens, pregnant women, mothers,\rfathers, caregivers and children in Alameda County. MPCAH enhances access\rto comprehensive, quality health care and focuses on early intervention and\rprevention services. The goal is to reduce health disparities, protect\rand improve health outcomes among Alameda County families, including pregnant\rwomen, parenting women and men, and their children.\r\rMPCAH programs work to ensure that women, children, youth, fathers, and\rfamilies achieve optimal health and well-being through the provision of\rclient-centered, culturally responsive, strengths-based services and supports,\rcombined with community transformation efforts that aim to improve neighborhood\rconditions.\r\rHEALTHY FAMILIES ALAMEDA COUNTY\rHealthy Families Alameda Countyis a voluntary home\rvisitation program designed to promote family and child health and wellbeing\rthrough a variety of services, including child development, access to health\rcare, and parent education. The program targets at risk pregnant women and\rfamilies with newborns under 2 weeks of age. Services are provided until the\rchild reaches 3 years. One of the two positions will be serving Spanish\rspeaking families.\r\rTHE IDEAL CANDIDATE\rIn addition to meeting the minimum qualifications below, the\ridea candidate will have experience conducting in homeand community\rvisits with clients, conducting outreach, administering screenings and\rassessments, and maintaining timely and accurate documentation related to\rclient needs and services provided. MINIMUM QUALIFICATIONS

Possession of a Bachelor’s degree and three (3) years performing case management duties for clients with complicated medical and social risk factors in a health care or social services setting. \r\rNOTE: The Civil Service Commission may modify the above minimum qualification in the announcements of an examination. \r

KNOWLEDGE AND SKILLS

EXAMINATION COMPONENTS
